Welcome to the admissions page for EOTAS Swindon. To ensure a smooth and efficient referral process, it is essential that all sections of the referral form are completed in full and with detailed information. This thoroughness enables us to understand each pupil's unique needs and provide the best possible support.

If the pupil has an Education, Health and Care Plan (EHCP), please include a copy of the EHCP as part of the referral. Additionally, an up-to-date attendance record, behaviour profile and recent assessment data must accompany the referral to give a comprehensive picture of the pupil’s current situation and progress.

Referrals received by Friday at midday will be considered at the EOTAS panel meeting the following Wednesday. Please note that it may be necessary to discuss a pupil on more than one occasion if the information provided initially is insufficient to make a fully informed decision.

For a referral to be processed, it must be signed with wet signatures by both the Finance team and the SENCO. Referrals without these signatures will not be accepted.
